RICHMOND concedes Ben Cousins is almost certain to miss the Round 1 clash against Carlton after severe stomach pain forced him back to hospital.
Richmond said yesterday Cousins was "touch and go" for Thursday week's clash against Carlton at the MCG and was in hospital for tests.
Cousins has been laid low for five days with the mystery ailment, which flared again after he came off painkillers at the weekend.
Richmond was hopeful Cousins would return to full training this week, but the latest setback means his hopes of playing Round 1 are slim.
The 31-year-old has missed the past twopractice games as well as critical training sessions.
The Tigers have fresh memories of Cousins breaking down in Round 1 last year, an injury that cost him six weeks.
Specialists have ruled out appendicitis, severe ulcers and pancreatitis, but tests showing inflammation in the stomach have yet to reveal an exact diagnosis.
Blood tests have failed to uncover a problem, with a colonoscopy likely after a gastroscopy (where a camera is inserted into his stomach) showed no clear problem.
Cousins was forced to stay in hospital again overnight to fast before tests today.
The club is confident Cousins, who has otherwise enjoyed a flawless pre-season, did nothing wrong to bring on the condition.
